That's a book handle scratch awl with its ice-pick point ground off and the remaining cylinder shape ground into a square, notched with a "+", and the ends of the resulting four little columns sharpened.

You can make one much like it from a phillips screwdriver. Use a file to create the four points. It’s a charm when working the bowl-shank area.
