Drilling the hole first and working "from it" will result in a more accurate center with the semi-sloppy lathes we all use. It also automatically compensates for any drill drift caused by the sort of long, angled wood grain found in dowel rods.

As for the polishing & etc., that enters into face-to-face demonstration territory. Dealing with matching levels & etc. is much of the reason why few people choose stem replacement and repair work. It's a pig bitch, and requires all sorts of adaptive, tricky techniques to do consistently and well. Trying to explain them in writing, though, would take hours. I'd end up having to write what amounted to an illustrated manual because of the many questions that a brief explanation would trigger.
