I am trying to understand how the stem work shown in this pic is done.

Are the individual pieces glued on to the stummel with a temporary internal rod for support and then turned on a lathe, filed by hand, or shaped on a sander?
Or are they attached to the stem?
Any ideas?

I can't speak to how this one was made, but usually, you will sand your pieces flat and epoxy them together, drill a hole in the shank extension and turn a tenon on the shank that the extension will fit over. Then epoxy it all together. So your shank will have a mortise for the stem and a tenon for the shank extension. Some people just glue the shank extensions on the end without a shank extension, but that's not very durable.
